8th Grade End of Year Activities

Deadline for Great America Signed Documents is May 8, 2017

Deadline for the Promotion Party Signed Permission slip is April 20, 2017

Please complete, print and submit the permission slip and health forms to Mrs. Mignano in the Counseling Office. By doing so you will indicate to the CW Administration that you and your parents have read and understand the details and the expectations for participating in the 8th grade end of year activities.

GREAT AMERICA: All students who wish to attend will be included in the trip, no donation is required to participate in the Great American trip offered by Charlotte Wood Middle School.  However, the cost to run these activities is substantial and it is necessary to solicit enough donations to hold this field trip. The recommended donation is $60 which can be paid to CWMS. Many donations were made at registration. If a student has a Great America season pass, we recommend a donation of $25 to cover the transportation only. If you donate now, please bring a copy of your receipt to the CWMS Office Manager, Mrs. Robertson, along with the signed parent permission slip and health form.

If you have any questions about any of the documents provided, please contact CWMS Assistant Principal, Mr. Corral at jcorral@srvusd.net or call 925-855-4415.

(If you want to chaperone (we need 20 parents), you must be approved through the District Volunteer System. To apply, please click here. Please email Mr. Corral if you are interested ASAP.)
